Un estudi de l'Institut d'Investigació en Atenció Primària (IDIAP) Jordi Gol avalua les estratègies per a la captació de dones amb un cribratge incorrecte de càncer de coll uterí, per tal de reduir-ne la incidència i mortalitat. L'estudi parteix de la base que en la situació de cribratge oportunista organitzat, que és el que es dona a Catalunya, només el 41,6 % de les dones tenen un cribratge cervical adequat.

El Grup de Recerca en Terapèutica en Atenció Primària (GRETAP) ha analitzat els criteris pels quals les persones d’edat avançada tenen errors de tractament farmacològic, o per omissió o per excés. La Prescripció Potencialment Inadequada (PPI) té més riscos adversos que no pas beneficis clínics, i és per això que la Dra. Inès Cruz, investigadora de l’IDIAP Jordi Gol, ha identificat les característiques principals de la PPI.

According to a recent study carried out by the IDIAP Jordi Gol, arterial calcification can be diagnosed by a quick and non-invasive method: the ankle brachial index test. People with arterial calcification are 2 to 3 times more likely to suffer from angina, heart attack, ischaemia and stroke.

A committee helps primary care professionals solve ethical dilemmas

The Health Care Ethics Committee (HCEC) of the IDIAP Jordi Gol was created in 2008 by a joint initiative of the Primary Care Research Institute-IDIAP Jordi Gol and the Catalan Health Institute (ICS). The HCEC responds to ethical dilemmas that arise in daily clinical practice. Consultations originate mostly from General Practitioners, and treatment and confidentiality are the most common issues brought forward.

Online clinical collaboration between primary care professionals and specialists decreases number of referrals.

The objective is to test if an Online Communication Tool between Primary Care and Hospital (ECOPIH) results in a decrease in referrals. With this system, primary care professionals should be able to consult specialists and get a response in less than two days.

The ECOPIH project consists in the design and implementation of a virtual platform with Web 2.0 technology that allows the online communication between primary care professionals and specialists. The project has been developed by the GP David Lacasta and the primary care nurse Souhel Flayeh, with the participation of physicians, nurses, social workers, physiotherapists, occupational therapists, podiatrists and dentists.

An intensive lifestyle intervention in primary care reduces the incidence of type 2 diabetes

The Endocrinology and Diabetes Area of the IDIAP Jordi Gol has been responsible for the implementation of the DE-PLAN project (Diabetes in Europe-Prevention using Lifestyle, Physical Activity and Nutritional intervention) in Catalonia. For the first time in Europe, the results in our country show the effectiveness of a lifestyle intervention in primary care clinical practice by significantly reducing the incidence of type 2 diabetes amongst participants at high risk of developing the disease.

As part of the European project DE-PLAN its Catalan counterpart, the DE-PLAN-CAT/ PREDICE project, conducted a prospective cohort study in primary care patients. A two-stage sequential screening firstly by means of the FINDRISC questionnaire (8 items that assess the risk of diabetes) and secondly with an oral glucose tolerance test (OGTT) was carried out in non diabetic patients 45 to 75 years of age. Primary care centres from around the Catalan territory have taken part in this study conducted from the IDIAP’s Research Support Unit in Reus and Tarragona

Prescription of biphosphonates before knee and hip replacement surgery is associated with a twofold increase in implant survival time.

A study led by an investigator of the IDIAP in the United Kingdom shows that prescription of biphosphonates to prevent bone fractures increases knee and hip implant survival. The study has been published in the BMJ.

Data from 41,000 patients with a diagnosis of knee or hip replacement surgery between 1986 and 2006 in the United Kingdom were included in the study. A subgroup of almost 2,000 patients who had been prescribed biphosphonates (medication to prevent bone fractures in osteoporosis) was identified. Implant survival was significantly longer in this group of patients than in those who had not received biphosphonate treatment. The study shows that the mean implant survival time was almost twofold in patients who had been prescribed biphosphonates for 6 months before or after surgery.

A programme that promotes physical activity from primary care shows promise after just a few months

This project analyses whether the collaboration between health professionals and graduates in Sport and Exercise Science increases adherence to physical activity. This pioneering initiative has obtained an award by the Catalan Society of Family and Community Medicine and has received support from the Primary Health Care University Research Institute-IDIAP Jordi Gol.

The study took place in 9 primary care centres of Barcelona and included a total of 384 patients. The programme was successful in increasing activity levels, quality of life and in decreasing the number of visits to the primary health centre (PHC). Physical inactivity is a modifiable risk factor related to the development of chronic diseases.

Consultations about fibromyalgia constitute 5 to 8% of all consultations in primary care. The typical patient is a woman of around 55 years of age, married, living with the husband and her children, with primary education, who is either a housewife or unemployed.

The multicomponent intervention (education and relaxation sessions) to treat fibromyalgia has been shown to be more effective than the standard treatment based on medication.

The FIBROQOL study included patients of the primary care centres of Gavà (PHC Bartomeu Fabrés i Anglada) and Viladecans (PHC Maria Bernades) with a diagnosis of fibromyalgia. The aim was to verify if a two-month multicomponent intervention improved the functionality of fibromyalgia patients.
