DE-PLAN’s objective is to reduce the burden of Type 2 diabetes (T2D) by implementing the European Guidelines of T2D Prevention Program in primary care settings throughout Europe for people at high risk, assessing its impact on baseline lifestyle habits and biomarkers, as well as its cost-effectiveness.

EURHOMAP: Mapping professional Home Care in Europe (800181)

EURHOMAP aims to fill the current information gap on home care in Europe by showing variations and models of provision and good practice in professional home care in 30 European countries. Focus will be on: organisation and provision of home care services; expenditures and financing; categories of patients and typical care needs met in home care; the role of the patients; policies and trends; and inventory of national sources of information on home care.

eHID: Electronic Medical Records for Health Indicator Data (2003129)

eHID uses electronically recorded medical data (EMR) collected during routine procedures in primary care to provide data on four health indicators (incidence and prevalence of diabetes, prevalence of ischemic heart disease and the burden of mental illness). This project studies the data collected to determine the best method of recording.


MOTIVATE: Motivational Skills Training in Health & Social Care

Many health and social care workers need motivational skills and enhanced general ability to support patients, particularly in cases of chronic disease and lifestyle changes, while at the same time they must able to support their own colleagues and peers.
This project aims to formulate a standard European Motivational Skills Training Programme to this end.


C.A.S.E. (Children of Addicts Study in Europe) (QLG4-1999-000105)

The objective of this project is to provide and evaluate longitudinal data on the children of heroin addicts and their parents from drug treatment services located in four European centres. Data will be collected in the form of standardized outcome measures from three different treatment modalities and an annually followed up during a three years period. The project aims to produce data and evaluation of drugs services which will lead to better and more coordinated transnational European policy decisions in the field of drugs treatment.

There is a lack of up to date comprehensive and comparable information on variation in development of primary care systems, and knowledge of structures and strategies conducive to strengthening primary care. This project aims to fill this gap by developing and implementing a Monitor on primary care, including system features, practice profiles and consumer evaluations in 30 European countries. Focus will be on: national characteristics; accessibility, continuity, coordinative capacity, comprehensiveness of services and consumer views and experiences.
